''ov Car'' in the reference: AYC #4 ov Car, means that this coin is overstruck on a denarius of Caracalla. ''ov'' for overstruck. I considered that this coin was overstruck on a denarius of Caracalla because on the reverse we see a remnant of laureate portraiture, indicating a denarius. The legend ANTONINVS begins at the bottom of the portraiture and this portraiture has a head shape close to what we see for Caracalla (small head). Obverse reads ..II COS IIII ... beginning above DRYA. This may correspond a lot of reverses for Caracalla. Furthermore, I add that a coin of his mother was over-struck with the same reverse die, see AYC # 4 Ju. Dom, RIC 2. Interesting point allowing to see which are the stocks of coins used for precise issues.
I did not notice a legend break on the reverse, the only part of the printed legend being EGINE (same case for AYC #4 Ju. Dom, RIC 2, using the same die). We can therefore neither conclude that a legend break is present, nor be certain that the legend was not IVNO REGINE instead of IVNONI REGINE.
